Skip General Information General Information Click here for general school information such as curriculum, dress code, summer reading, etc. Course categories | HEALTH FORMS FOR STUDENT ATHLETES Ohio High School Athletic Association (OHSAA) requires parents of students in grades 7 through 12 who will participate on a school athletics team to have a current Pre-participation Physical Exam form on file prior to the first practice of their sport(s). Please complete pages 1, 3, and 4. The physician completes page 2. Send the completed form to Columbus Academy, Attention: Kim Stercula, Athletic Trainer. If you would like more information about the form, click on the link, "Frequently Asked Questions regarding the OHSAA Consent Form," to take you to OHSAA's website. Pre-participation Physical Exam Form ADDITIONAL FORMS FOR STUDENT ATHLETES Please use the Prescription for Athletic Training Services form if you take your student-athlete to a physician for an injury evaluation. The form will inform your physician of the rehabilitation facilities we have available to your child free of charge at Columbus Academy. The treating physician should complete the form, return it to the athlete or fax it to the private fax machine in the athletic training office, 509-2522. The form will give our athletic trainer the documentation necessary by law to treat your child.
